i need help understanding my epstein-barr virus test result. i have been very sick for over a month and doctors are sure its mono, but 2 mono tests were negative. on ebv test i see that only ebv vca igm is elevated. is this a positive result?

Probably not EBV: IgM antibody tests are very unreliable; your positive result likely is false -- if you really had EBV, your mono tests would be positive. You may have "heterophile negative" mononucleosis, i.e. mono not caused by EBV. Probably not serious -- mono (whether due to EBV or otherwise) almost always clears up over a few weeks with no long term health consequences. Discuss with your doctors.

Insufficient: VCA IgG and EBV IgM sometimes are not sufficient to tell, what exactly is going on. More tests may help, for example "D early antigen (EA-D)" and "Epstein Barr nuclear antigen (EBNA)".

Yes: Epstein-barr virus is the cause of the disease infectious mononucleosis, or "mono".  Nausea and vomiting don't occur in every case, but if liver enzymes such as alt are elevated, are fairly common.

Makes perfect sense: Igm antibodies rise only during the actual, acute infection. They then fade away and are replaced by the various igg antibodies, which are the ones that provide lifetime protection against the same strain of the virus. So your numbers are consistent with the fact you had mono years ago. There is no igm, and positive igg levels. So you are immune and do not currently have mono.

Depends on 4 results: Once infected, a person keeps his EBV virus forever in his body. EBV is common. There are usually 4 antibodies tested. A patient should have a reason before being tested (he has some illness). If positive for past EBV, that just means a past infection. If positive for current EBV, that means a current infection. If both, it could be a re-activation of EBV, or a resolving "current" EBV infection.

Cause of mono: Ebv is one of 8 human herpes viruses and is the classical cause of infectious mononucleosis (glandular fever).  Once infected, an individual will asymptomatically have the virus in saliva intermittently and can transmit it to others not yet infected.  Not everyone who get infected have symptoms.
