May not be related: Need to know why the spleen is enlarged. The symptoms can be related to the reason the spleen is enlarged. The cyst  may not be a problem. The treatment is to treat the reason the spleen is involved. Sometimes the spleen will need to be removed

Hepato-splenomegaly: Hi Keith: an enlarged liver and spleen (also called, hepatosplenomegaly) has many potential causes. Broadly: infections and cancers. For someone in India, a few possibilities that must be considered include TB and malaria - there are other infections too. For malignancies, evaluations for lymphoma's and leukemia's.  She must seek care as soon as possible.

Further tests ?: An ultrasound may help. Liver function tests can rule things out. Other viruses can cause this. If increasing in size, this will need definite evaluation and further work up.

Not with all viruses: Mononucleosis does cause splenic enlargement but all viruses do not cause splenic enlargement.But there are others beside the Ebstein Barr Virus which causes mononucleosis.Cytomegalo virus causes it.But you say it get enlarged whenever he is sick.I suggest you discuss with your pediatrician if it is of concern.If it goes down after his illness .then I see no problem.Discuss with his Pediatrician

Many: Spleen can enlarge for a dozen different reasons. Some are benign like chronic liver disorders(cirrhosis) or it can be a blood disorder like leukemia or itp like conditions. Only a hematologist can help you sort it all out.

Not an emergency: If the pain is bearable and you are able to eat without vomiting and sleep without any sign of fever etc. you can wait a few days to see your doctor for a definitive assessment. This may require some blood testing and or imaging study to confirm finding and figure out cause. If not feeling well in general, you should seek immediate assistance at your local hospital ED.
