Hypothyroidism: Elevated TSH strongly suggests that hypothyroidism is present which, if untreated,  can produce numerous problems such as fatigue, weight gain,  constipation and impaired thinking.  Rarely the cause could be a pituitary tumor.

TALK TO YOUR DOCTOR: If you do have true hypothyroidism you may need to be on it life long. Discuss it with your doctor and you may need follow up blood tests  to determine if this is temporary of lifelong.

Need repeat panel: A repeat TSH and thyroid panel including T3 (liothyronine) and T4 should be checked and verified.

Elevated TSH: The elevated TSH suggests your thyroid is in the process of failing, probably from Hashimotos thyroiditis, which is autoimmune. If you feel well, you need not take anything now, but your thyroid will likely fail, and you will need to take levothyroxine. This could happen in 3 months or could take years.  Many doctors would start meds now. Diet/herbs/iodine/vitamins will not help.

if so, quick rise may be that your
1. Taking it at same time as other meds
2. With food

should be first thing in am 45 min before eating or other meds

may have changed company making it
use brand name synthroid

took wrong dose.

Nothing: The TSH is more accurate. A free T4 is more accurate than a t4, which can be affected by  many factors such as estrogen. Are you on any form of estrogen, bcp's etc?

No: The immunoassays for the two tests are distinct.

Cushing syndrome: Cortisol thru the roof is Cushing syndrome until ruled out. Our job is to figure out why this is happening. Taking steroids can do this but shouldn't affect thyroid. I'd be more concerned about pituitary adenoma which can cause both Cushing AND central hypothyroidism (usually low TSH means hyperthyroidism) but in case of space occupying lesion in pituitary, it may put out ACTH in favor of TSH.
