You should discuss with your prescriber your concerns.  If thinking of a suicide plan, tell them as well.  It's not a pleasant nor sure thing and other help is available if this isn't working.

The secondary amines like nortriptyline and desiprimine have less of the antihistamine effect.

D/C it, or diet: Elavil (amitriptyline) often causes wt gain. If it could be replaced with another antidepressant, then have your md d/c it and replace it - but you'll still need to do some exercise to help lose the wt. If you can't d/c the elavil, (amitriptyline) then you have to be aggressive about wt management: watching caloric intake and doing vigorous exercise. In worst cases, sometimes an appetite suppressant is used too.

Number of conditions: Although originally the first tricyclic antidepressant, it was later employed to prevent migraine headaches, and even later used for trigeminal neuralgia, postherpetic neuralgias, and the pain of peripheral neuropathy.  Actually, may be fairly efficacious, but anticholinergic side effects are intense and may well limit dosing.

Caution: While the 10 mg dose of Elavil is low, combining with Lexapro (escitalopram) increases the risk for serotonin syndrome and  irregular heart rhythm. Also you are taking two antidepressants. If Lexapro (escitalopram) is not working, discuss with prescribing doctor to consider trying Elavil alone or another class of antidepressant. Be sure you have careful medical supervision while on these meds. I hope this helps.
