No: If you take a pill each day, regardless of timing, you would have effective protection.  The likelihood of spotting and breakthrough bleeding would be increased if you do not take the pill about the same time each day, but the contraceptive efficacy would not be decreased.

First pill pack: If you started your pills on the very first day of your period, you are totally protected. Based on the contained ejaculation, assuming no  intercourse without the condom, you are protected

Pregnancy?: Sexual activity with male clear discharge is not a protection against pregnancy.  Even if the male does not completely ejaculate, there may be sperm released. If not on birth control pills, and your "period is late", get a serum HCG test  which is very accurate to be certain you are not pregnant.

started birth control pill day 10 of cycle. my boyfriend and i had sex on the 14th day. no ejaculation, possibly precum. took plan b within 5 or 6 hours. continued taking my birth control as instructed. (levonorgestrel and ethinyl estradiol) chances?

Possible pg.: Plan B was a good thing to do.  But it is not perfect.  Continue to use condoms till on BCP's longer.  As the package on Plan B states. Do a pg test in about 2 weeks.

Yes: Depending on how long you have been on birth control pills i would say that you are absolutely fine. Especially since there was no sexual intercourse, also since you took the pregnancy test 16 days later this should give you a very clear answer as to if you are pregnant or not.

Fibroids: It may or may not increase the size, but if the pills are controlling bleeding, it is appropriate.

Reduce symptoms: Birth control pills can be used to decrease the menstrual cramps and quantity of bleeding caused by uterine fibroids because they decrease the production of female hormones, prevent ovulation and help regulate menses. 
But, birth control pills do not reduce the size of uterine fibroids.
Fortunately, there are number of surgical and minimally invasive options for treatment of uterine fibroids.

Have MD sample it: Have your MD take a sample or biopsy of the endometrium or fluid to have an analysis done of the content to rule out any evidence of abnormal cells.

Maybe: Penicillin has nothing to do with getting pregnant; having unprotected sex may cause pregnancy, especially if you have missed a bc pill. Take a pregnancy test, and get prenatal care immediately if positive. Penicillin will pose no threat to the pregnancy, but any underlying infection may.
