Shouldnt: Does it burn every time?  Burning is a sign of pain that something is wrong and should be checked out if it is a recurring problem. Have you ever had herpes? It sometimes can act this way.

Yeast medication: By mold in the vagina i assume you are referring to a yeast infection. This can be treated with over the counter yeast medication like monistat. It is ok to use it while you are on your period.

Clot?: Sometimes when blood clots the plasma portion of blood can look like tissue... And then this can get embedded in a clot and look super scary.  I'm guessing that's what happened.  Sometimes lining from the uterus can be funny looking too.  Rarely there could be a polyp that falls off (unlikely at your age). If you're sexually active, could also be a gestational sac from miscarriage.
