High end: The average respiratory rate @ 6 would be expected to run 18-28 / min with a rise of 5/min for every degree of fever (f) .This would put a rate of 40 at the higher end of expected. If the process giving her a fever is located in her lungs, it could easily push it higher. I often get chest x-rays in kids more due to increased work of breathing than what i hear on exam.
