Varicocele's Ache: Yes. The swelling of the varicocele leads to pain. The ache is usually short lived as the trauma that led to the inflammation subsides.  There is a surgical option if the pain is intolerable. But, active varicocele's can hurt.

If both testicles and no swelling or discharge, it&#x27;s more likely referred pain from the prostate area, usually not due to STD of any kind. See doctor.
