i am a experiencing a tremor like feeling internally. i have no external tremors that i can see. also for the last week i have had dark dreams that wake me up at 4am everyday. no noticeable stiffness or slowness. parkinsons possible?

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated.

Generally yes: The rate of progression in Parkinson's disease varies among patients. Currently, however there is no good way to distinguish between rapid and slow progressers. In one study, 25% of patients were seriously disabled or dead 5 yrs after onset. This number increased to 80% at 10 to 14 yrs. A few patients were found to have limited diability at 20 years, but this was not typical.

Parkinson's : Parkinson's will make people move slower and slower.  It can also make them have trouble getting out of a chair or walking around. But by all means don't let them live the rest of their lives in a chair!  it is important to continue exercising as tolerated.  They will probably get tired, and won't be climbing as many steps as before, but i encourage them to do whatever they can. Exercise will help their parkinsons, and also help their heart.  As soon as people become chairbound, their cardiac health changes drastically. One thing to remember is that parkinson's medications can make people very sleepy -- they may sleep much more than before.

PT/OT: Range of motion, resistance, strength and balance require attention. A good physical & occupational therapist will be able to train you. You have parkinson's for 40 years and you are currently 47 yo?

why can t healthy dopaminergic neurons be moved into the area of parkinson s affected neurons to treat parkinson s disease ?

Accepted Answer

Not so simple: Most of the dopaminergic neurons end in the substantia nigra of the midbrain, originating in various deep brain structures. These nerves are already affected by the disease. Unaffected nerves of course still remain but remain connected to brainstem. So outside nerve cells (from another source like stem cells are needed) nerves don't like to be moved around anyway, because they often die.

Not yet: Neurotransplantation as a treatment for parkinson disease reached the stage of human trials over 15 years ago, but the field, which is still in its infancy, has encountered a number of roadblocks since then, both political and scientific. With hope that stem cells may be used as a new source of dopaminergic neurons to replace the degenerating nerve cells in parkinson disease looming.

Pigmented nuclei: The dopamine loss involves predominantly the basal ganglia area, but also involves dorsal vagal nucleus, substantial nigra, and areas of any dopamine interaction within brain and brain stem.  But, to be complete, Parkinson's is a multi-neurotransmitter degenerative deficiency syndrome with other neurotransmitter systems involved.

Accepted Answer

Yes: Although the causes are multifactorial and sometimes poorly understood, there is a hereditary component.  Keep in mind, that even though a parent may have it, it does not mean that it will be necessarily passed on.  The tendency, however, may be there.

Sort of...: There are certainly diseases and conditions that damage the same areas of the brain as Parkinson's disease, most notably kernicterus (consequence of severe neonatal jaundice).
