Cigarettes: The best data appears to be that cigarette smoke decreases sperm count and quality somewhat in men with known varicoceles (varicoces veins of the testicles).  Marijuana and alcohol have been suggested but in my clinical practice their effects are limited except at very high usage levels.

No: It does not. Premature ejaculation is just that premature. It does not have any impact on the quantity of sperm. It may however have impact on fertility, which requires the sperm connecting to the egg-in this case timing is everything.

No, not normal: Average testicle size after puberty is ~2" long, 0.8" width, and 1.2" height (5 x 2 x 3 cm). Causes of a small (atrophic) testicle include: 1) painful conditions like torsion, infection, and severe trauma, and 2) painless conditions like the presence of a varicocele, abnormal development, genetic anomaly, prior surgery affecting blood flow, steroids, etc. Depending on cause, could affect fertility.

Masturbation: Sperm are created in the testicles. Masturbation is a common part of the human experience and in reality does not lower or raise the sperm count.  You are just fine as you are.

Unclear: Propecia (finasteride) blocks the conversion of testosterone to its active form called dht. While testosterone and not Dht are necessary for sperm development, it still not a good idea to take it if you are trying to get your partner pregnant.

Propecia (finasteride) and Semen: Yes.  Propecia (finasteride) very well may limit the production of semen and lead to a less robust ejaculate.

Generally not but...: If this were a common problem there would be far fewer babies born here in the santa cruz, ca area :) 
however, there is evidence that heavy marijuana use may lower sperm count somewhat and reduce the ability of sperm to fertilize eggs, but no evidence it causes "infertility" per se. But for couples experiencing fertility issues it would be wise to reduce or stop marijuana use. See comment:.
