what causes this and how long before it goes?

It is genetic, usually due to a European ancestor, can be controlled well with diluted tea tree oil to kill Demodex that can make it worse and Doxycycline 20mg per day. why would a dermatologist assume a malar rash is rosacea and not lupus because it is not a raised rash? i have read lupus can appear as flat lesions?

Rosacea vs lupus: Malar rash of lupus is flat or mildly raised.

It spares nasolabial folds.

Difficult cases may need a biopsy and immunoflorescence can clinch the diagnosis

have rosacea, tried metronidazole,, cerave, & vani cream. all make skin red, itch, hot, burn, hurt. i see a dermatologist but it's not helpin. :(?

Rosacea: There are other treatment options.  Oral medications such as doxycycline for the pimples are very helpful.  If you have excess redness, a topical medication, Mirvaso can reduce the redness for 8-10 hours at a time.  Laser therapy is also effective to reduce the redness
