See your doc: Penile lesions can range in etiology from inflammatory, infectious, or malignant causes.  There are also other possibilities.  See a primary care physician and get it checked.

Rashes: Hi, it’s hard to say without a picture but there is a great app that may be able to help you get a diagnosis without being seen in person by a dermatologist. The app is called VisualDX and is one of the best, if not the best out there. Many clinicians, including myself, use it. Just snap a photo and it gives you a list of possible diagnoses. Good luck!

Rash vs lesion: Both psoriasis and lichen planus can give penile rashes, that may have few symptoms.. Less commonly, candidiasis and seborrheic dermatitis can also give persistent red patches. Most of these would improve with one week of otc mild cortisone cream. If no response, a skin biopsy should be done, as rarely pre-cancerous lesions can also look like this.

Sounds like: Early balanitis and should be evaluated and treated by a physician before it encompasses the entire head of the penis.

Could be: genital wart, so it is best to show it to a doctor for accurate diagnosis and proper treatment.  Dermatologist or urologist would be able to help you.

Penis lesion: Fleshy lesions of the penis may be venereal warts. These are caused by the human papilloma virus and are sexually transmitted. Have you had unprotected sex?  An examination by a dermatologist or urologist is recommended for diagnosis and treatment.

Foreskin should B OK: Foreskins do not wear down. Foreskin can be pushed back, but if too tight may remain "stuck" behind glans. This is paraphimosis and needs urgent attention, hence foreskin should always be pulled back over glans. You may need to consult a urologist or else send us a photograph of your penis for us to review privately so we can comment upon it in confidence.

can i have balanitis without pain/discharge? masturbation makes head have reddish patches. foreskin reddish. patches go away until masturbate again.

Skin and Sex: It sure sounds like you have friction caused skin damage and i wonder if you are not using any lubrication.  If you are using lubrication then you may be reacting to that lubrication. But, in most cases men need some lubrication during regular and repeated sexual activity.
