: Any known trauma?  The tissue of the tip of the penis is quite delicate. I strongly recommend being examined &amp; advised by a urologist especially where you're experiencing difficulty retracting your foreskin.

STD: You could have syphilis if you are sexually active and had sex with  a partner who has multiple sexual partners. Syphilis can start as painless nodule in shaft of penis which can ulcerate later.
See your doctor to have it checked and do appropriate test and treatment.

Can be normal: If there bleeding in the subcutaneous space (between your skin and the muscle of the belly wall) this can track down with gravity into the penis, scrotum, or down the thighs.  If he develops marked swelling, pain or passage of bloody urine, these are reasons to see your surgeon.  Icing the area may provide some comfort.

i had a varicocele surgery done 5 days ago and i think it's healing alright but now there is a painless dark red, bruised like spot that's a little swollen on the top of my penis at the base. what is this?

: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. You may watch and wait. If the swelling is subsiding, it may just be part of the healing process. If you get a discharge, the swelling gets larger, you develop fever or pain, it would be prudent to see a doctor. my son was born on monday and circumcised in thursday. his penis naturally has some swelling and is a bit bruised, however the one side of his scrotum also looks very bruised (dark purple / black). is this normal?

Scrotal bruising: Penile bleeding is common after the circumcision, but bruising of the scrotum not so much. I think it would be of value to have your provider double check the scrotum. Rare, but some scrotal infections have developed and turned into serious illness.
