MRI and : Most other imaging studies can only visualize, when they can, anatomical or physical problems.  They usually cannot determine functional problems.  For example, if there is scar tissue around a nerve which is causing pain, imaging usually cannot tell where these are.

Other option: A sprain/strain is when a ligament/tendon is injured by overstretching. There may be minimal or quite a bit of damage to it. Ice massage (24-36 hours), elevation, gentle rom, motrin/advil are the keys to maximize healing.

Differing Opinions: I am not sure why you would worry about a temporary diminishment of sperm production when you are hurting.  Ice is good in an acute injury to diminish swelling. But, once swelling has occurred ice provides no benefit.  Heat helps soften the muscle spasm and helps with some minor pain control.

Muscle Health: Yes. Of course. Also using muscles by moving or exercising; stretching out when strenuously using muscles.... Sleeping well.  So lots of good reasons to health. Now specifically you seem to be having pain much longer than any other man i know with a minor scrotal or cremaster muscle injury. If you have not gotten to the doctor monday is coming and perhaps it is time.
