If there is a partial tear in the inner layer, then a high pressure of jet of blood entering this tear can cause the layers to separate.  This can potentially occlude (block off the artery) which can cause a heart attack.

: A garden variety carotid artery dissection tends to cause early on, cerebral ischemia, and an eventual full-blown stroke may occur.  The presence of anti-coagulation might prevent some of the localized thrombosis and mitigate the immediate damage potential but might result in hemorrhage in the area of cerebral infarction.  So, an unpredictable two-edged sword?

Yes,not as above : Vertebral artery dissections can cause bleeding usually from the dissection/rupture of a saccular aneurysm. These aneurysms are more common intracranially than in the extracranial location. The aneurysms are typically located at branch points of the major vessel. Treatment of the aneurysm is typically endovascular, with the placement of  coils to occlude the aneurysm.
