Obstructed airways: I think context would be helpful in order to answer this question satisfactorily.  Air trapping is a symptom or sign of obstructed airways where the air has an easier time getting in than out.  Conditions like COPD, Emphysema and Asthma are the usual causes and can be seen on xray as flattening of the diaphragm and on spirometry as a increased Functional Residual Capacity.  Other causes are rare.

Smoke free: The best bet is to avoid exposure to cigarette smoke & high levels of ambient air pollution. There is no one size fits all answer for this - it depends on each patient's clinical profile and individual characteristics.

Still controversial: Small airway is considered the primary defect in asthma but one cannot diagnose the presence of asthma by small airway obstruction alone.  In fact it is often present in seemingly well controlled asthma. I presume that your fvc and fev-1 were normal and thus would not lose sleep over it.  COPD is also not diagnosed by this measure. Air pollution can cause this problem as well.Prognosis is good.

CO: Carbon monoxide is lethal, you cannot have a steady exposure to it. Respiratory problem results from air pollution, no matter what kind it is.

Hard to tell,: With those findings, further testing is needed. Unlikely to be COPD, given your age, please keep the follow up with your doctor / pulmonologist for continuity of care and better management, take medicines as prescribed, good luck

Stress Test: For this test you will not have to run or walk on the treadmill. They will give you medicine to elevate your heart rate. The machine they will use to take pictures of your heart is wide open and will not enclose you. I have had this test myself several times . If you are really nervous your doctor can give you a medicine to relax you before the test.  You will do great.

Both: Both conditions can cause wheezing and respiratory difficulties. Best option is to start stopping smoking which would benefit both. Abn ihaler would help asthna and wheezing.
