What do you do?: Do you work outside? Cold shop? Need a bit more info. Think about everything that is different between working or not and you will probably find the reason.

every night 35 y/o male, penis head turns blue/purple and my penis and testicles are freezing cold. it has also caused a form of ed. testosterone is 651. had ct with contrast. vascular dr said everything was fine. urologist said it will pass. help!?

: Unless you've had some injury to nerve endings or spinal cord pathology, I wouldn't be concerned about those symptoms. I would take the advice of your urologist.

Testicle Pain: Testicle discomfort usually comes from inflammation, not infection. Antibiotics are useful, but alone don't always help.
I suspect you have prostatitis as the cause.  Prostatitis can refer pain to the testicles, penis, groin and more. 
Increase water intake, reduce caffeine, begin nsaids (aleve) with food twice a day for two weeks. Hot soaks in a tub helps. F/u with md if not better in 7-10 days.

Okay...: Where your testicles position is within the expected normal variation and will have an adequate fertility like most men around. Remember: No two persons have ever born exactly the same and no one has been ever born perfect, but always within the range of "normal" variation. So, live on and enjoy life.

No: The spermatocele is, by definition, a cyst arising from the epididymis which is directly attached to the testicle.

Less likely...: Unlikely for anatomic shrinking to occur. Your tissue cannot regress in that area. It might be your perception but that's hard to tell. Make sure that you are still able to get a normal erection at the normal girth and length. Any chance you lost weight recently or are dehydrated compared to before?

Probably...: What you saw and described could have been there for all your life but just recently and incidentally discovered leading to inciting your curiosity and anxiety. Those black dots could be just blackheads of dried plugged sebaceous glands, which should bear no harm and no need for active care as long as being comfortable.
