Antibiotics/ steroid: A topical otic antibiotic and steroid drop may be indicated. Flonase AQ over the counter, may also be helpful. Please see your physician for follow up. Watch for signs of fever and pain.

i have a sinus infection which makes my head and ears full.  usually take claritin d 12 hour - it helps but quit and tried mucinex.  not good. help,?

Sinusitis: if it has been over 7-10 days you may need antibiotics. you can also try afrin but do not use longer than 3 days. you can also try a nasal steroid like nasonex which are available over the counter now. lastly, try nasal saline rinses and warm sinus compresses.

BP Concerns: The decongestant in Claritin (loratadine) D can be helpful for acute facial pain/pressure and congestion but has side effects which can be significant. If you have a history of blood pressure issues or other cardiac problems I would not recommend. Consider talking with Allergist about other possible treatment due to refractory disease. Drying agents like ipratroprium bromide sprays and methscapolomine can help

: No, it drains in your nasal cavity. Ear ringing (tinnitus) can happen when you have a blocked eustachian tube so if you have symptoms of rhinitis due to virus or allergy, claritin-D may help. If you do not have any nasal symptoms, claritin-D is less likely effective and you should see an ENT doctor for a thorough evaluation if tinnitus persists.

Possibly: Sometimes different people react differently to otc meds, the Claritin (loratadine) or the pseudofed can cause the headache. Try the claritan with out the "d" and see if the samething happens. But if your headache continues to be significant, consider eval by your primary doctor.

Infection: If you do have bacterial infection, you will need to have antibiotics prescribed. Claritin (loratadine) d is used for symptomatic treatment and doesn't treat any infection. Viral infections usually go away without treatment and will require symptomatic treatment of which Claritin (loratadine) d is one. Your doctor needs to decide what to do next if Claritin (loratadine) d is ineffective.

Not usually: Claritin (loratadine)-d can cause elevated BP in those who already suffer from hypertension.  Claritin (loratadine) can cause arrhythmias (disorders of heart rhythm) and so can the "d" part, pseudoephedrine.  If you have hypertension or heart problems, you should stay away from most decongestants, most people can use antihistamines such as plain Claritin (loratadine) safely however.

: Treating nasal congestion also involves the cause of the nasal congestion. If you have allergies, discuss testing and avoidance measures. The use of HEPA filter unit may be useful and keeping your home dust and mold free. Avoid pollen, pollution and smoke. 
Rinse nasal passages with saline to remove irritants and to keep nasal passages clear, after checking with your PCP.

Unrelated: not a known side effect of either of the drug components, you want to check with your doctor for another reason, which may be simple neuritis (nerve inflammation) can occur after viral infection, best wishes
