No Leakage, no worry: If the condom functioned as it was supposed to, there is no worry.  If any semen entered your vagina that is a different story, but as stated, you should be fine.

When was Plan B : used. Plan B One-Step is up to 89 % effective when taken within 72 hours (3 days) after unprotected sex. It can reduce chance of pregnancy by up to 95% if used within 24 hours. It continues to reduce the risk of pregnancy up to 120 hours (5 days) after unprotected sex, but is less effective as time passes. A correctly used condom will improve efficacy beyond those numbers.

Depends: It depends on whether the condom broke or not. The risk is obviously much higher if the condom broke and you were using no other method of contraception. The risk with a condom that doesn't break is 2-3% ( the risk with hormonal methods used properly is less than 1%). If the condom broke it is the same as without anything. That is approximately 25%. Best to use a more reliable method AND condoms

Chances very low: Your chances of pregnancy are very low with condom use, and women normally don't ovulate when they are menstruating which makes your chances of pregnancy even lower but not zero.  If you are having symptoms of pregnancy and don't have your period next month, you should check a pregnancy test.

Condom: well condoms have an overall failure rate of 20% if used regularly because most leaks are large enough for you to detect.  Pulling out probably does reduce that failure rate.  Always consider using a spermicide like VCF (vaginal contraceptive film) in case of small undetected leaks and if this is a regular partner, its probably time to consider a birth control pill or Depo-Provera provera (medroxyprogesterone).

Oral BCP ?'s: If you are punctual in taking the bcp's, and have not missed starting a new pack late (one or two days or so after the end of the placebo pills) the risk of pregnancy "w/o condoms" is minimal, but condoms do help prevent STD's.
