Usually if the symptoms are significantly affecting the quality of life and conservative measures have failed then surgery will be required. Also the age of the person and associated medical conditions will impact the decision. Typically progressive severe central canal stenosis ends up needing surgery.

Try Non-operative : First treatment option would always be physical therapy/rehab to strengthen core muscles groups.  If that fails then epidural and or/facet injections using either steroid or plasma rich platelets.  If that all fails then surgery would be last resort.

For patients with refractory symptoms or severe disability, surgery is probably the best treatment. This involves removing the protrusion. Need for fusion is high.

In your case the problem will be on both sides.
