Yes: What you see on the outside, a 'pin head' size lesion, may be enormous underneath where you cannot see, inside the tooth. We see this all the time. A radiograph is used to determine the extent of the decay. It may be so deep that the nerve has been affected, and it seems that way from your symptoms. Go see a local dentist for treatment.

Unlikely: A single localized lymph node is a sign of a local problem. I would look at other causes: teeth, mouth infections. However, it is a good idea to check HIV test.

Continuous chains: Lymphatic system absorbs and carries things too big to go into the arteries and veins like bacteria eaten by white cells and digested food. Lymph nodes "taste" the lymph to see if anything bad is there like bacteria/viruses/cancers, etc. They enlarge in response to the inflammation they create when they react.  They are all connected so they cannot just break off, it may have shrunk or pushed deep.
