Jock itch?: Most common thing would be a fungal infection in the groin. Try lotrimin (clotrimazole) over the counter. If no better in a week, see a doctor.

Knot : If the knot is on the skin,  then it may be an epidermoid cyst.   If it is deeper inside and separate from the testicle,  then more than likely it is a benign finding.  He should visit with his doctor to be certain.

Warm soaks: Likely an infected hair follicle or sebaceous cyst. F it heads and drains with soaks it doesn't need antibiotics, but if if progresses it needs to be seen by an md.

Pruritus genitalia: This is one of the neurodermatoses often triggered by stress. The more you scratch, the more you itch!. Try applying a cool compress wet to dry for 3-4 minutes and then apply OTC hydrocortisone 1% ointment. If this stil is not helping, you need to consult a dernatologist for stronger topical steroids and possibly oral antihistamines.

Testicle: Wouldn't worry too much, but should be checked out.  Might be just a simple diaper rash, or can be a skin infection.

He may: have have renal  ( ureteric ) stone some will not have pain , besides other causes. Take him to a urologist to investigate the cause.

Scrotal irritation ?: Exposure during sexual relations with a partner who may have a yeast (fungal) infection or other infection can cause a contact dermatitis.  There may be other causes (friction secondary to prolonged sexual activity).

Get it checked: It's difficult to say what your problem could be without visualization of the rash.  It's best to follow up with a provider so they can evaluate your condition and determine it's seriousness or lack there of.

Consult with Dr.: This should not be diagnosed over internet. Some dr.S will comment on emailed photo + phone call others need to examine in person. Could be hydrocele, inguino-scrotal hernia, trauma, torsion of testis or appendix testis, epididymitis or epipipymoorchitis, cyst of epididymis & virtually never tumor. Pediatric urologist or surgeon can diagnose in seconds.
