Location, location: Where on the hip is the pain. A quick primer. Hip joint is often groin pain, side of the hip is usually bursitis. We of coursenneed  more information. You can discuss this with your physiciasn or ask to see a rheumatologist.

Hamstring: The best stretching for this is your hamstring muscles (back of your legs)  do this daily to reduce sciatica.

Core conditioning: You can perform all kinds of stretches for the core muscles as well as as the extremities.  Sciatica is typically made worse with deep bending of the lumbar spine or flexion of the hips (bringing your knees up towards your head).  Do these exercises slowly and you should improve your sciatica slowly. Avoid exercises that make any numbness or tingling worse.

Disc Hydration: Disc are more fuller in the AM after a nights rest due to the absorption of fluid overnight.  Thus fuller discs can herniate more and cause pain in the AM, then as the discs lose fluid throughout day they do not compress the nerves as much. Also the reason the AM is the worse is because the discs do not irritate the nerves all night while sleeping and start irritating upon waking.

Possible: It is possible you have a radicular pain. That happens when there is inflammation in the spinal canal that causes pressure in the nerve that exits the canal at that level and causes pain. Commonly people call it 'pinched nerve'. Sciatica is a type of radicular pain. Hip pathology should also be considered.

Neither: At age 23, neither  is likely.  Sciatica is due to compression of nerve in low back, usually causes pain down the leg. It's rare at your age. Injury to coccyx can cause your symptom, as can muscle and joint injury there. For previously undiagnosed cancer to present in this fashion in a young man would likewise be VERY rare.

my doctor told me i have sciatica but i have no back pain just buttock and leg pain which is more intense at night and early morning why is that?

See below: Many people with sciatica experience just buttock and leg pain without back pain. Sciatica will often calm down on its own however if it does not after a few weeks or if you experience symptoms of weakness in the leg, or any problems moving your bowels or urinating you should see your doctor right away.

Siatica: Most LBP or sciatica is not serious and goes away with time, physio, and simple analgesia.  Warning signs include numbness, weakness, urinary or defecating problems, or fever.  If you are older or have a cancer that is concerning also.  If any concerns or symptoms persist you should see your doctor.  If symptoms persist and sleep is an issue tricyclics may be worth discussing with your doctor.

Lumbar DDD: you may have lumbar degenerative disc disease with spasm. you need to see your provider and may need an MRI.

i have sciatica and have been literally sitting on a heating pad for hours at a time.  could this be making the pain worse?

Accepted Answer

Probably not: You would want to make sure you are not using the heating pad to the extent it is causing a burn or other soft tissue injury to the area.  In general, topical heat or cold are ok to try, but usually don't have too much affect on patients with sciatica.  This is because the root of the problem is deep in the spine at the level of the nerve roots, and tooicL heat or cold does not change core temp
