Palpitations: Could be normal, but a longer monitoring period such as a Holter study, an echo, CXR and thyroid studies would help to confirm this. All the best.

my son is 28 years old, and just started drinking iced coffee.  everyone he drinks it, he gets lightheaded. could caffeine be the cause?

Yes: Just stop for a week and see if symptom improves and then add back ice coffee and see if symptom comes back

Hmmmm . . . .: There's a small amount of caffeine in green tea, but i'd wonder about possible other causes for feeling constantly lightheaded.For 3 months. Easiest way: The easiest way to determine whether caffeine is causing or exacerbating anxiety is to stop using it for a time -- and see if symptoms go down.  Be aware that if you're accustomed to drinking many cups of coffee, caffeinated soda, energy drinks, etc daily,  you may have some withdrawal symptoms for a couple of days.  Headaches, flu-like symptoms, tiredness, etc. Tapering could be better.

Constant HA: Stopping caffeine  abruptly  can cause withdral ha, uu don't need to stop caffeine u can. Cut it down to two cups a day.

Not unheard of!: Very uncommon, but occurs.  Why have you been off of coffee? I might give you a better answer if i knew.  However, this can occur with caffeine.

Would not hurt: You may be very sensitive to the effects of caffeine. At your age these effects should wear off without consequences. To be secure about this see a doctor.

Physiology!: Great physiology question!  When performing a valsalva maneuver, pressure in your chest rises which impedes venous return to the heart and leads to a decrease in stroke volume/cardiac output.  This can lead to a transient feeling of dizziness.  The effect should only last seconds.
