One can use over the counter meds if  no medical reasons that prevent use. Bulging discs do not cause pain, they are normal findings on mri.

An uncomplicated disc protrusion can be treated with a minimally invasive outpatient microdiscectomy - go get checked out.

The psoas plays a role in back pain and overall stability of the spine, it can cause compression of the spine and sacroiliac joint when in severe spasm. Try upward dog or cobra positions with deep breathing to see if it helps to alleviate component of your pain that's muscular.

However, gangreen can exist without tetanus germs & have a variety of other germs that are just as deadly.

Either should heal with cleaning and a week or so, and some topical antibiotic jelly if penis.
Both are vascular tissue and gangrene is unlikely.
