Keratosis Pilaris: Keratosis pilaris is a common skin condition in which a protein in the skin called keratin forms hard plugs within hair follicles.
Treatment may include:
moisturizing lotions to soothe the skin and help it look better, 
skin creams that contain urea, lactic acid, glycolic acid, salicylic acid, tretinoin, or vitamin d, 
steroid creams to reduce redness.

Sounds like hives: Urticaria, hives, are caused by exposure to something the body responds to in an allergic response.  Pink raised areas that blanch when pressed, itchy is a typical hive.  Anxiety can also be a cause. It could be from something that came in contact with upper thighs, perhaps new pants, new detergent, new scent. Most cases, cause not found. Rx with antihistamines. If very bad, may need rx w steroids.

Bumps: This is probably a common skin rash caused by the rubbing of pants on the skin and sweating. You say he's had it for a long time so unless he's been taking a certain medication for about the same period of time its not a drug eruption (allergy). Pants made of a different material, something softer and more  permeable, may help. Ask a dermatologist to take a biopsy of the bump to be certain.

Painful bumps: Painful skin lesions in a dermatone raises the suspicion of herpes zoster(shingles) infection. If this is the problem it is treatable and,thus, I recommend an evaluation by your physician or a dermatologist.
