Lumps "that count" (dominant masses) tend to be distinct from normal breast tissue.  They may be fluid-filled (cysts) or solid (tumors)--this can be differentiated by ultrasound.  Tumors may be benign or malignant--this can be differentiated by biopsy.

Hard, dense: Breast tissue can come in many different patterns according to breast size, fat content and age. Many patients have lumpy breast.  As  rule, physician exam is necessary for all lumps!  i usually have my patients feel the back portion of their hand between the thumb and 1st finger. The muscular flesh is like normal breast tissue.  Then move over to a knuckle..This is what a breast lump feels like.
