Don't Worry: Sometimes women have unusual menstrual cycles.  You can have some irregular bleeding or spotting before or after your menses or even mid-cycle.  It is probably a good idea to start keeping a calendar of when you bleed, spot, etc. to show your OB/GYN.  Take ibuprofen, naproxen, or Acetaminophen for the cramping for the next 24-36 hours.

If you doubt the result you can repeat it in 1-2 days. If you were pregnant the hormone level would rise enough for detection. If negative  see your doctor for irregular periods.

PCOS: With PCOS you do not ovulate regularly. When you don't ovulate you don't produce progesterone which stops the uterine lining from growing and makes it get mature. If the lining just keeps growing, it gets so thick that parts start breaking of and you start spotting or bleeding. Over time, this un opposed growth can increase the risk of cancer so you need to see you doctor for treatment.

Irregular bleeding is  one of the most common problems Obgyns treat.: If this is one time problem it usually resolves on its own. If this is a recurring problem you should be evaluated. Infections &  hormonal changes can cause spotting. If there is a concern for pregnancy then do  a home pregnancy test .  there are many options available to help regulate the menstrual cycle. Schedule a visit.

Yes they can: Fibroids are benign (non cancerous) tumors of the uterus that are associated with painful, heavy periods.  Depending on their location, they can certainly cause bleeding unrelated to your cycle.

They can be treated with medications that are hormonal or non-hormonal, and they are also commonly treated surgically by removing the fibroid (s) or the entire uterus.
