Unlikely: Unless there was trauma during the actual exam i wouldn't expect such a finding especially if there was no bleeding immediately after the exam.

Absolutely: A fetal pole is expected to b e seen with a mean sac size of around 16mm (transvaginal sonogram). As such, you may just be too early to see it and patience here is warranted.

Depends: Spotting may be normal early in pregnancy. You should call your OB's office and let them know.  They may decide to schedule an appointment if it persists.

When was sonogram: If the sonogram was done before you began having spotting and pain then it probably should be repeated. If the sonogram was done after the episode of bleeding and spotting then it sounds like things are going ok so far. Keep your follow up appointments with your Obgyn to help guide you through pregnancy.

Gestational sac: Unfortunately a sac with irregular contours is not a good sign, and the gestation may not be viable.  However, ultrasound at 5 weeks can be tricky and if everything else is OK a follow up scan in a week or so may be more definitive.  Talk to your OB.

Maybe: Unfortunately a dropping bhcg is not a good sign. Your doctor is likely to want to recheck the hormone level in a few days to confirm.

Cycle control: Irregular periods are one of the most common problems obgyns treat. If this is one time problem it usually resolves on its own. If this is a recurring problem you should be evaluated. Infections ;  hormonal changes can cause spotting. If there is a concern for pregnancy then do  a home pregnancy test .  There are many options available to help regulate the menstrual cycle. Schedule a visit.

Hormonal imbalance: Menstrual irregularities are not uncommon and anovulatory cycle is a common cause. If you miss more than two periods it would be time to see your doctor. If you do not wish to be pregnant, use contraception all the time. You may consider implanted contraceptive or an IUD.
