But, clinically, it would most likely represent a long-existing overgrowing hair-follicel-related sebaceous gland, which is harmless and requires no Rx except superimposed infection may occasionally kick in. So, wait and see if being comfortable; but, see Doc timely if in doubt.

Get exam: For peace of mind have an examination by a dermatologist or urologist. It's not possible to render a diagnosis without inspecting your penis.

Normal: Blackheads may occur anywhere there is skin. These occur due to hair follicles becoming blocked with oil or buildup of dead skin cells.

I assume you: Mean in the area of the blackhead, not that you have an erection. That would be important on social media, but not here. Use warm compresses on the area; keep it clean; and if it doesn't heal normally, check w/your physician.

Could be any: Lesions on the penis casn present in multiple forms. It can be any of the lesions listed. He needs to be seen by a urologist or dermatologist.

My : My concern is that these lesions you are describing may not be acne at all. 
Have you seen a dermatologist for this problem? Was a diagnosis of acne made? What you are describing could be in fact condyloma aka genital warts. These often look like acne, black heads in particular. If that is the case, then be assurred there are very good treatments that can eliminate these lesions.
On the other hand you may be dealing withsomething else, possibly a dermatitis due to some type of allergic reaction or allergen you are exposed to.  Very difficult to say without seeing the lesions myself.
I hope you will visit a dermatologist for this problem asap. You should not have to deal with something that is ruing your life.
