It sounds like the original culprit was the whitehead that maybe then progressed and the blackhead was the scab that crusted over it which was replaced finally by scar.

If more help is needed, retinoids such as Retin-A or Differin gel can be used with or instead of benzoyl peroxide. Oral antibiotics such as tetracycline can be used also.
