Back pain, nausea : If it is sudden, more likely the spine. There are several abdominal causes that feel like back pain. E.G. Inverted uterus. Severe pain anywhere can cause nausea.

Pancreas : Pancreatic symptoms can lead to this presentation.  Referred back pain is a common finding.  Given the fluid loss, and inability to drink, i highly recommend a visit to your local emergency department.

Any bowel disorder?: You might have a gastroenteritis. Any history of diverticulitis? Any fever? Stay hydrated and if the pain does not improve you might need a GI evaluation.

Family member: needs evaluation. Many causes of abdominal pain and emesis. Acute abdomen or obstruction has to be ruled out. Take family member to urgent care or ER or PCP ASAP
