ASCUS: The preferred approach for evaluation of women ages 25 or older with ASC-US cytology is testing for high-risk types of HPV with triage of women who test positive to colposcopy. Use of HPV testing to triage further evaluation of ASC-US is the most effective strategy for detecting high-grade premalignant disease or cervical cancer.

It is a very unusual: Pap smear report. I don't believe this one finding indicates that cancer is certainly present ... but it definitely needs clarification and raises suspicion.  Please discuss with your performing physician/gynecologist. At a minimum, I believe the smear should be repeated and consideration given to additional diagnostic testing. Good wishes.

Lots of variety.: The term "spindle cell" sarcoma is a generic descriptive term that doesn't really give enough information.  It would be like asking, "i saw a convertible, how fast can it go?"  additional testing often reveals exactly what kind of sarcoma, and then once you know that, you can more accurately discuss the prognosis and the treatment.

Yes it often is: Spindle cell cancers are often called sarcomas. They can arise either in the soft tissues or in the bones. Ask your doctor what is the name of your cancer? If it is a sarcoma, chemotherapy is useful in about half of the cases, often it is used in conjunction with surgery and radiotherapy.

Some help: High grade spindle cell sarcomas are aggressive and every effort to remove the primary, control the surgical site if larger than 10 cm with RT should be made.  If metastatic foci are noted surgical resection can also benefit as seen with lung mets.  Primary treatment with drugs like Ifosfamide and platinum may help but primary removal is most important

Not always cancer: Spindle cells (sc)  have bilateral tapering poles (like a football); their presence on pap smear indicates issues ranging from benign reactive or reparative changes, artifactual/normal squamous cell shape, to rare malignancies (carcinosarcoma, metastatic desmoplastic melanoma). Sc morphology is seen in connective tissue. Pap grade (high or low intraepithelial lesions, cin 1-3) is more tellig.N.

To help diagnosis: Pathologists often use immunohistochemistry to help in arriving at a diagnosis.  For softn tissue tumors immunohistochemistry can help better characterize the type of tumor...For example if is smooth muscle derived or a neural tumor etc.  In terms of nodular fasciitis, it can morphologically appear similar to some smooth muscle tumors and some other fibroblastic tumors so ihc may be used.
