This is serious: I&#x27;m glad you&#x27;re on benzoyl peroxide. Adding an antibiotic may help some; I would suggest simple tetracycline. However, this is unlikely to clear up real cystic acne. Especially at age 22, acne is a grave social and professional liability. You need to consider isotretinoin, even if you&#x27;re read &quot;how horrible it is.&quot; It is worth the risk, used with proper guidance. Best wishes.

i have severe cystic acne. i was taking 40mg accutane .i'm down to just 20mg. if antibiotic is added would that help? if so which one ?

See a dermatologist: If the cysts appear with your periods, this is likely hormonally driven, and your doctor may consider medications to stabilize hormonal fluctuation.  If cysts are rare, he/she might inject the cyst with a steroid to reduce inflammation.

Probably: Since junior is on accutane, he's under a dermatologist's care and the decision should be made by him/her. Sometimes the rapid death of the bacteria from antibiotics in cystic acne can itself be troublesome. I'm very glad junior's getting effective treatment; thanks for making it possible.

Cyst: no antibiotic is the best and most of the time cystic lesion like you describe may need to be lanced and drained, try some warm compresses also, it may help for cyst to come close to the skin surface and drain on its own

Isotretinoin: Better late than never. Cystic acne won't be cleared by antibiotics. It's not for the faint-hearted but it's probably  what you need. The internet is full of scare stories but the fast majority of folks with cystic acne are glad after they've done it.

Acne: Possibly a birth control pill may help the hormonal changes that accelerate acne growth. You may also want to look at some dietary changes that would be helpful.

Keep at it.: There are three main parts to approaching acne...1) reduce the amount of bacteria on skin.. 2)  keep pores open...3)  reduce oil production.  Antibiotics can be topical and/or oral.  Other surface treatments aim to reduce bacteria and/or keep pores open.  Keep experimenting and working with your doctor until you find the right mix.  It can take a few weeks to see improvement even with right mix.
