have anal tear, can it start to bleeding again after it stopped day before? can pain from it get worse when it wasn't too bad to begin with? and why?

Yes: The bleeding can be intermittent. Avoid hard bowel movements, take over the counter stool softener and soak your bottom in warm water for about 15-20 min. The pain can increase the spasm of the sphincter muscle which inturn can cause more pain specially during a bowel movement. If symptoms are not getting better you need an evaluation by your physician amd possibly referral to a colorectal surgeon.

Proctologist: I believe you should get a much earlier appointment with a proctologist. I believe you will receive as good or better care for an anal fissure than from a colorectal surgeon. However if it's a complicated anorectal problem,then consult the colorectal surgeon after. . Lubricate the anal canal with glycerin before your BM. Take stool softener if stools are too firm. Soak anus in warm water 3 times

Anal fissure: Sitz baths can help. Sit in the tub for 20-30 minutes at least twice a day. Apply a Lidocaine ointment after the bath. Also make sure you are on a non constipating diet which contains lots of fiber.If after treatment you are not improving see a gastroenterologist to rule out an irritable bowel syndrome.

Maybe, but...: Bleeding per rectum may originate in the anal canal, the rectum, or further up the line. Frequent anal sources of bleeding include hemorrhoids & fissures, that may respond to modifying stool texture (fiber, softeners) and topical agents, or require further intervention. Similar bleeds can be seen with anal & rectal cancer, infections, ischemic colitis, diverticular bleeds, inflammatory bowel, etc.

Depends: If the fissure is acute and shallow, correction of an abnormal bowel  issue will resolve it in 2-3 weeks. A chronic fissure may stop bleeding but may not heal without surgical correction. You need to be properly evaluated with a discussion of your diet, fluid intake, bowel pattern and confirming the diagnosis with followup to confirm actually healing even if the symptoms resolve. At 23  not likely : but Squamous  cell carcinomas  of anal canal mimics  anal fissure , could be missed if biopsy is not done  in long standing  cases and in older age group  ( not at  23 ) .  Anal fissure of  5 years is too long speak to your doctor for alternative therapy  svn surgery and biopsy of the area .

Absolutely!: Aspirin and all the other NSAID drugs (ibuprofen, naproxen, etc.) can thin your blood and make bleeding worse.  If you have an anal fissure you should avoid these drugs, and see a doctor about the fissure.  It's a painful thing to have and there are treatments!

Might need treatment: Has he been seen by a doctor for this?  Keeping from straining, avoiding hard stools, and the use of topical nitroglycerin (a prescription) or diltiazem should be considered as first-line.
