can mental issues cause physical pain? my anal fissure was totally healed in august. when i feel mild pain, i visit dr. and he says you are 100%ok. a little anal spasm give me shock of anxiety & depression and pain starts to increase. what is this?

: PTSD or recall of symptoms prior to healing is likely contributing to your symptoms as you are unable to differentiate the pain from the spasm from the pain from the fissure. Talk to your doc about ways to address your anxiety.

can a digital rectum exam damage the nerves or muscles in/around the anus/ rectum? i had one done around 5 days ago and now have spasming and pain.

Spasm: Absolutely, spasm can reduce blood flow and cause pain/aching in the leg. Not sure why you had the steroid IM shot though. Consider getting your low back looked at by a chiropractor or a DO who knows how to manipulate.  Odds are you have a structural issue somewhere below L2.  Soak in a hot bath then do some gentle stretching exercises.

Several things: The first is to have a sigmoid or colonoscopy. Magnesium 200-400 mg before bedtime would be good to try as it relaxes muscles. I have had success with a digital exam  working on relaxing the muscles in the area. The other is called Neural Therapy or nerve blocks to the sacral plexus.

It may well take: It may take a visit to the doctor to get some relief. That's because it takes one's primary care doctor's exam to figure out what the cause of the pain is. After that, the doctor can offer some pain relief. Pain usually means some damage is happening, such as tissue inflammation. So, first the exam and diagnosis. Secondly, the treatments and relief. Then, the pain in the butt should resolve.

can i use triamcinolone inside my anus for hemorrhoid relief ?

Yes: Triamcinolone is stronger version of hydrocortisone which is steroid commonly used in over-the-counter hemorrhoid creams. But as Dr. Simpson said (https://goo.gl/8fvAPm), 1st make sure it's just a hemorrhoid and not something else. Also plan on eating more fiber, drinking more water &amp; exercising regularly (all good advice anyway) to reduce constipation &amp; need to push hard which aggravates 'rrhoids

4 persistent fissure: Could U be referring to fissures, fistula, abscess? Sexually transmitted disease (syphilis, gc, hiv, etc.) should be considered. Hemorrhoids &amp; rectal prolapse can appear as an outward bulging. Anal trauma may cause mechanical injury. Skin eruptions need to be considered. Once diagnosis confirmed, chronic/recurring fissures may respond to prescription nitrate or diltiazem topicals. Surgery 4 some.

Topical meds: You should ask your doctor if you can try nitroglycerine cream on your fissure.

Talk to your MD: Talk to your doctor again.  You may need a different medication to treat the fissure if its not healed.  It could also be a reaction to the cream itself.

Sitz baths twice daily and avoiding constipation may also help.
