Hypothyroid: A TSH level of 12.26 indicates that you not on a sufficiently high dose of thyroid medication and should talk to your doctor about increasing the dose.

Adrenal fatigue: No - not the least because there really is no such thing as adrenal fatigue this 'condition' has been manufactured by purveyors of supplements to treat a constellation of symptoms - none of which actually respond to the supplements.
Alopecia is primarily a genetic or infection problem and can be normal as in receeding hairlines or as a result of acute stress which can cause 'telogen effluvium'.

i have terry nails, fatigue and hair loss. i'm only 26, don't drink or take meds/drugs. is this likely due to bad diet or should i get blood work done?

Terry?: I am not sure what terry nails are, but you could be describing some symptoms of low thyroid. I recommend a blood test to rule this out.

tsh was 1.8 in aug, 2.2 in dec, and 2.6 last week. fatigue and hair loss continue to worsen. tpo ab 10 iu/ml. is "watchful waiting" my only option?

Thyroid not the: cause. Your thyroid function is not responsible for your symptoms. It is not feasible to say what is causing the symptoms without examining you. It would be appropriate to seek a second opinion.

Fatigue etc.: Without delay you should notify your doctor and make an appointment with him/her as soon as possible. 
If there is cause for delay in an appointment a visit to the Emergency room would be in order.

Beyond the basics: Simply tell your doctor about all ts you have and she/he will also ask you several questions before a physical exam is done. 
What happens next will depend on the exam and the lab data.
It may be controlable by your doctor or might need a referral to a Neurologist!

Rearrange life: You may have to think about giving up your job or take a lighter schedule in school.

Lots going on here: You have a lot of symptoms - some physical, some mental.  It sounds like you should get a thorough check-up to see if there is a problem such as hypothyroidism, medication side effect, depression, or something else going on to cause all this.

Tinea capitis: You may have a fungal infection, but not sure why you're so tired.  Have the dr, check you out.  Best wishes.
