Mental confusion(Confusion)

Recurrent episodes of altered mental status with confusion, disorientation, or sudden loss of bladder and bowel control, limb numbness/tremor; each time hypotensive, tachycardia, hypokalemia. Last 1-2hrs before returning to baseline. What could it be?
