The cause is unknown.
As it tends to run in families, there may be a genetic or familial link. Some suspect an alteration in the immune response as many patient who have psoriasis also have a geographic tongue. It can also be idiopathic meaning we do not know what causes it, it is usually self limited and does not worry me as an ent.

Geographic tongue appears to be a benign event of the top(Dorsum) of the tongue.
In teaching or addressing tongue hygiene your scrapping or the use of the tooth brush to clean(debride) that surface of accumulated bio load. GT does not seem to respond to this type of care. It appears to be involved with a sloughing of the surface lining of the tongue and produces the look of outlined form .
