In brief: Trigger points

These are caused by damage to the muscles and superficial nerves.
Injections of subcutaneous mannitol over the nerves and/or acupuncture can be very beneficial in correcting this problem.

In brief: Many causes

Trigger points develop in muscles from many causes including trauma repetitive use, stress, radiculopathy.
Trigger points are focal areas of muscle fibers within a muscle that are hyperirritable. They are felt to be small areas of contracted muscle within taut bands of muscle. When these trigger points are pressed they cause pain not only locally, but can cause referred pain at a different sight.
