Hard to say. Lung cancer often spreads to the adrenal glands, but adrenal tumors are often benign. The best predictor of how well your friend will do is how quickly the tumor is growing. If he truly had nothing there in march and now, despite chemotherapy, has large adrenal tumors growing that is not a good sign. If, however, your frined has had adrenal tumors all along, then the prognosis is much better.
Very Poor. Sad to say, your friends prognosis is grave. He probably has months left. Very sorry.
Metastatic disease. Anytime cancer spread from the lung to other organs, the patient has a limited life expectancy. How long he or she can survive depends on multiple factors. The degree of weight loss is the leading indicator.

Genearally- months. Unfortunately, in general, the average prognosis is in the range of months. It depends partly on functional level: is your friend resting most of the time or still up an around? Cosider localtherapy. I think the lung cancer can metastasize to the adrenal gland either by the lymphatic or the hematogenous route. In case of metastasis by the lymphatic route, it can be a solitary metastasis, and the prognosis can be relatively better.
