In brief: One visit crown
He can characterize it with a furnace (if needed) and they are absolutely incredible and quite esthetic if the dentist is well trained. So you don't have to worry about a second appointment with another shot.

In brief: Not always
If tooth has rootcanal done and temp crown placed then many times topical anesthetic gel will work.

In brief: Numb gums
I usually don't, sometimes if there has been no root canal on a tooth, the tooth is very sensitive, depending on what cementing procedure it may need numbing.
Topical is usually not enough. Injections are what may be needed. It really depends on your sensitivity level.
