Documentation and medical records of the present disability is critical. Make sure that the condition that you have is eligible and qualifies as disability. Understand that there are different claims for disability depending on your situation, i.e. VA/military, social security, state; and different criteria may exist for each type.

If you have real injury, and is seeing a trustworthy physician, with documented medical findings, then he should have no trouble getting a successful disability claim. Problems occur when there are totally subjective complaints with no objective findings, discrepancies in the history or contradictory multiple examinations by different providers, dr. Shopping, etc.
