In brief: Mainly hearing loss.

The primary symptom is a gradually worsening hearing loss that occurs in one or both ears over many years.
As hearing loss progresses, ringing or similar noise in the ears (tinnitus) may also worsen. A small percentage of patients may also suffer from episodes of vertigo/dizziness; however, most patients only experience the hearing loss from otosclerosis.
