Soreness in bone behind ear, what could be wrong?

Tendonitis. In the absence of ear pain or hearing loss, the most likely cause of pain behind the ear is tendonitis of the scm muscle which attaches to the mastoid tip. Hear and analgesic should help.

No. A knot or lump in that area is probably an enlarged lymph node. Lymph nodes enlarge due to infection/inflammation typically. I only worry about cancer if nodes continue to grow over time.
