In brief: 1-3 weeks
The mold of your teeth should take no longer than 30 minutes.
Then the mouth guard will need to be made. If the mouth guard needs to be sent to a lab, it probably will be done in 2-3 weeks. However if it's just a protective mouthpiece, some dental offices can have it done within a day.

In brief: Depends
Depends on whether it is a sports mouth guard or a Night Guard for grinding/clenching, your appointment availability, Dentist's scheduling availability, appliance's material/design/complexity, fabricating laboratory’s turn-around time, etc.
Best answer, ask your Dentist. Remember that Dentistry is a service, not a commodity. Every patient receives unique individualized care.
