In brief: Not often, but....

In this young age group, benign causes, such as hemorrhoids are by far the most common cause of blood in bowel movements, however this is always a diagnosis of exclusion.
Anyone with blood in the stool needs to be worked up, as cancers in the GI tract can occur at any age, and of course if there is a family history of colon cancer, especially at a young age, routine surveillance should be considered.

In brief: Rare

In this age group, bright bleeding with boowel movements is almost always ourtlet type from an anal cause such as hemorrhoids or anal fissure.
Brisk diverticular bleeding may present this way as well. By the time a polyp is large enough to cause bleeding, it is usually already a cancer which is not very common in those under 45. Any dark, tarry or black stools may be from stomach and needs evall.
