In brief: Estrogen
Premenopausal women are protected by the estrogen their ovaries make.
The exogenous estrogen taken after menopause doesn't do the job. There are many theories as to why endogenous estrogen is protective but beyond the scope of this forum. I have never seen a premenopausal women with a heart attack unless she smokes or has diabetes. You can take that to the bank!
