In brief: Depends

There are various grades of abnormalities in pap smears.
Mild abnormality, especially if you are hpv negative, may be followed by repeat pap. More severe abnormality may prompt colposcopy and biopsy. Follow your doctor's advice.

In brief: Mostly

Pap smear abnormalities can be of different levels.
A mild abnormality known as ascus without presence of high risk hpv virus does not need a coloscopy. Most other abnormalities of pap with low or high grade dysplasia(pre cancerous abnormality) will require a more detailed exam i.e. Colposcopy and possible tissue sampling /biopsy.
