It's a bloodless injection using a fine needle (before injecting, staff should always withdraw the plunger to make sure the return is clear).
Initial doses are very small (they do build up to 0.5 cc in the end). Subcutaneous space is a "potential space" that can accept the volume (doesn't hurt as much as intradermal). Ask for staff you like. Topical anesthetic like lidocaine/prilocaine may help.
